What is personalized medicine?

Personalized medicine is a healthcare model that emphasizes time, unrestricted communication, and preventive care. Patients pay a membership fee that allows for enhanced connection with the physician, unhurried appointments, and a more proactive approach to health.

How is personalized medicine different from traditional primary care?

This high-connection subscription care model is detached from dependence on insurance reimbursement and thus outside the time/frequency/scope of insurance plan restrictions. Our model uses patient investment in extraordinary care beyond plan coverage to provide ongoing exams and communications to achieve health optimization. Unlike traditional practices that rely on high patient volume, our practice limits the number of patients, allowing Dr. Kofler to perform annual and follow-up diagnostic routine exams focused on your wellness, overall health and optimizing health outcomes, supported by year-round 24/7 communication support, to help you achieve your best possible health outcomes.

What types of health concerns do you treat, and how are they managed?

Routine care: Wake up with the flu, a rash, tick bite, UTI? Book a same day in office or telehealth visit so Dr. Kofler can take care of you right away – regardless of where in the world you are. We promptly diagnose and treat a wide spectrum of conditions—so you can get the care you need without a trip to the ER or multiple specialist visits.

Chronic conditions: Long term conditions like hypertension, diabetes, obesity are managed through individualized plans and emphasis is placed on prevention of chronic diseases.

Mental health: Your mental health is an essential component of your overall wellbeing. Whether you’re feeling off or experiencing anxiety, depression, insomnia, or other complex symptoms, you will be afforded the necessary time, compassion, and treatment to improve your quality of life.

Sexual health: No topic should feel uncomfortable to discuss under our inclusive, comfortable, friendly environment. From family planning, STI testing, erectile dysfunction to menopausal symptoms, we are eager to put you at ease.

Urgent concerns: Unexpected injury or illness at the most inconvenient time? Need a prescription refill? No problem – Dr. Kofler is available to you at your convenience.

Do you accept insurance?

Infinity Care is not designed at this time to bill insurance plans, so, all of the care we provide is covered by subscription fees.

Our model is tax-advantaged: it qualifies for employer/self-employed deductible funding, HSA/HRA/FSA/MSA funding. Please check with your tax expert or plan coordinator to confirm. 

What does the membership fee include?

Membership typically includes:

  • Unrestricted, unrushed appointments
  • Direct communications with your physician by phone, text or email in support of achieving routine exam goals
  • Annual and follow-up routine diagnostic exams not restricted or limited by insurance plan terms designed to optimize your health
  • Preventive and lifestyle-focused care
  • Medical care navigation and oversight

Can I use my commercial health insurance and/or Medicare for blood work, imaging, or other tests if I’m an Infinity Care member?

Yes. Your Infinity Care membership covers your physician’s time and services, including comprehensive visits, preventive care, care coordination, and direct access to your physician. It does not cover laboratory testing, imaging, medications, durable medical equipment, or services provided by outside facilities.

When medically appropriate, we will order blood work, imaging studies, and other diagnostic tests just as any primary care physician would. The laboratory, imaging center, or other outside provider will bill your commercial health insurance and/or Medicare (if applicable) for covered services. If your health insurance/Medicare does not cover a particular test or only covers part of the cost, you may be responsible for the remaining balance.

In some cases, if a test is not covered or is only partially covered by your commercial health insurance or Medicare, the laboratory’s cash price may be less expensive than billing through insurance. If that is the case, we will let you know so you can make an informed decision.

Your Infinity Care membership and your insurance and/or Medicare benefits complement one another. The membership fee pays for the personalized physician services and enhanced access that insurance/Medicare does not cover, while insurance/Medicare may continue to cover eligible laboratory tests, imaging, and other medically necessary services ordered as part of your care.

Is personalized medicine worth the cost?

Many patients find personalized care invaluable for the stress-free assurance it provides. The focus on longitudinal care, prevention, and early intervention can reduce unnecessary emergency visits, hospitalizations, and long-term health complications.

Can I still see specialists?

Yes. Dr. Kofler will coordinate referrals and collaborate closely with specialists to ensure seamless, informed care.

Do you offer same-day appointments?

Yes, same-day or next-day appointments are a cornerstone of concierge care and are offered whenever possible.

Can my family join?

What happens if I need care after hours?

Members have enhanced communication with their physician outside of regular office hours for urgent medical concerns, providing peace of mind when it matters most.

How do I join?
